Background: Knee Arthroplasty involve the replacement of knee joint with different materials including metal alloys, polyethylene and ceramic materials. Patients generally had positive expectation regarding improve function and quality of life regarding, but several factors were play a key role in denying the decision of arthroplasty. Objectives: The Aim of the study is to find different patient concern that play a vital role in denying the decision of total knee arthroplasty procedure in advance osteoarthritis patients. Methodology: It was a prospective qualitative study conducted at Department of Orthopedic outpatient Department of Saidu Group of Teaching Hospitals (SGTH) Swat, from July 2023 to December 2023 (Six Month duration). The study includes 119 patients who were advised total knee arthroplasty for advance osteoarthritis and different reason of refusal were identified through structure interview. Results: Five theme of denying total knee arthroplasty were identified; the most common factor for refusal for Arthroplasty was the patient concern about cost and affordability. Among 119 patients, 34.4 % (n=41) patient refused arthroplasty due to financial unaffordability. Moreover, 26.9 % (n=32) patient refused due to Elderly surgery awareness gap, 16.9 % (n=20) patient refused due to fear related to surgical Complications and duration of rehabilitations, 12.60 % (n= 15) patients declined to proceed with arthroplasty due to the stigma associated with previous treatment failures experienced by relatives or acquaintances and 9.20 % (n= 11) patients refused autoplastic procedure due to culture and religious beliefs. Conclusion: Different factors contributing to the denying arthroplasty procedure include socioeconomic factors, fear of complications, fear of surgical failure, and social and religious concerns, underscore the multifaceted nature of decision making in healthcare. Overall, understanding and addressing these factors are essential for optimizing patient outcomes and promoting equitable access to advanced arthroplasty procedures.
